May 2026 Graduation – Name Change Deadline for Candidates

The last day for a candidate to submit a name change request is April 10. Any requests submitted after this date will not be processed in time for the Commencement Program nor diplomas.

Candidates should fill in the Change of Name form and submit it with all appropriate documentation. Instructions for submission as well as a list of acceptable documents are found at the bottom of the form.
