All University clubs and organizations are required to submit an event approval form to the office of Student Development and Leadership four weeks before the event. Event Approval Form

For all events, an event reservation set-up form is due to Jackie Fee no later than two weeks before the event. Event Reservation Form

Any changes that need to be made to your original requests must be done through an activity registration addendum form and submitted to Jackie Fee no later than one week before the event. Addendum Form
