Immunization Alert

On June 28, 2002, Governor Mark Schweiker signed into law a requirement that students enrolled in institutions of higher learning residing in residence halls or other university-owned housing be vaccinated against meningococcal disease.

 

Students who wish to be exempt from this requirement must meet the following conditions: students (or parents/legal guardians in the case of students under the age of 18), after having been advised of the risks of the disease and the availability and effectiveness of the vaccine, must sign a written waiver stating that they have reviewed the information and have chosen not to be vaccinated against the disease for religious or other reasons.

 

It is MANDATORY that ALL full-time residential students submit a completed Health History and Immunization Record to Health Services. These forms will be mailed to students prior to New Student Orientation. Forms can be returned by mail or brought to Orientation weekend.
