Wagoner Nursing Medal Nomination Form

To nominate a student for this award, please fill out this online form by April 1, 2013. If you have any questions, please contact Karen Thompson, R.N.C., M.S.N., or Phyllis Krabill.

If you are a nominee, please submit:

• An essay about your nursing philosophy and service activities
• Two letters of recommendation. At least one letter of recommendation must be from an IU faculty member. (A faculty nomination will be considered a reference letter.)
• Current curriculum vitae or résumé

Please send all documents to Karen Thompson by April 1, 2013. Incomplete nomination packets will not be considered.
